The Major Twist: The impoverished, disabled janitor is actually the legendary "Miracle Surgeon"—the man who invented a cardiac surgery technique that saved the lives of over 1,000 children, as well as the U.S. Secretary of Health. The Secretary of Health and the National Medical Board appear, immediately revoking the villain's medical license for life and arresting her on the spot.
